Request for Updated I-20/DS-2019

You should submit this form if you need an updated I-20 or DS-2019 for any of the following reasons:

To report a change of major (Undergraduate Students only)
To report a change in funding (updated financial evidence/letter must be uploaded)
To replace a lost/damaged/stolen I-20 or DS-2019
To request a study abroad I-20/DS-2019 or global co-op I-20/DS-2019
To add or terminate a dependent
To report a name change
