This form is for current students to request an updated I-20/DS-2019 due to a change of major (undergraduate students only), change in funding, to replace a lost/damaged/stolen document, to request a study abroad I-20/DS-2019, to add or terminate a dependent, or to report a name change
Request for Re-entry
This form is intended for students requesting an Initial I-20 after a status violation, an Initial/Transfer I-20 after an academic dismissal, or for students returning from a leave of absence of more than 5 months and/or who are not eligible for Reactivation.
